If it is a concern to ensure that your insurance provides coverage prior to the initial treatment, our medical billers will verify your insurance for you. Please complete and submit the following form (at least 24 hours in advance please). After completion, we will contact your insurance company for you and one of our acupuncturists will get back to you prior to your first appointment!
